What it is

CLI represents end-stage PAD. Without restored flow, amputation risk within a year is high. Timely revascularisation is the intervention that most changes outcomes.

Symptoms & signs

  • Constant foot or toe pain, worse at night
  • Non-healing ulcer or gangrenous toe
  • Foot pale on elevation, purple when hanging down

Diagnosis

Urgent Doppler, angiography, and toe pressures.

Treatment

Angioplasty and, where needed, stenting of tibial vessels below the knee. Coordinated with the wound-care team and, in some cases, plastic surgery for coverage.
