Deep Vein Thrombosis

Sudden severe leg swelling and cyanosis — phlegmasia — go to emergency.

What it is
Most DVTs are treated with anticoagulation alone. A minority — iliofemoral involvement, phlegmasia, or young active patients — benefit from catheter-directed thrombolysis or mechanical thrombectomy.

- Unilateral leg swelling
- Calf or thigh ache, warmth
- Discolouration

Diagnosis
Compression ultrasound of the leg veins.

Treatment
Anticoagulation for most. For extensive proximal clot: catheter-directed thrombolysis, mechanical thrombectomy, and — when a compressive lesion is found — stenting of an underlying May-Thurner narrowing.
