What it is
In a dissection, blood enters between the layers of the aortic wall. Type A (ascending) is a surgical emergency; Type B (descending) is often managed medically first, with endovascular repair (TEVAR) for complicated cases.

Diagnosis
Emergency CT angiography is the reference standard.

Treatment
Uncomplicated Type B: strict blood pressure control and surveillance. Complicated Type B (malperfusion, expansion, refractory pain): TEVAR — a covered stent to seal the entry tear.
